For After-Hours Urgent Care:
If you have an urgent issue outside of regular hours, please call our main number at 773-275-2586. You will be connected to a primary care provider who can assist you. 



For Emergencies:

IF YOU NEED IMMEDIATE ASSISTANCE OR HAVE AN EMERGENCY, DIAL 911 IMMEDIATELY.

Do not use the Participant Portal for urgent or life-threatening emergencies.
